We’re here to guide you through the first steps of becoming a client. Like any healthcare service, starting with MARI requires completing important forms and permissions. We’ve designed this process to be as straightforward as possible while ensuring we collect the necessary information to begin your care.
This request for services form is the first step. This form helps us gather essential details such as contact information, your concerns, insurance coverage, and any custody agreements (if applicable).
Who should complete the form:
- New clients filling it out for themselves
- Parents/guardians completing it for a child under 18
